Lipedema, Hormones, and Gut Health in Endurance Sports

Marília Marques and Bruna Mahn explore the role of estradiol, the gut microbiome, and the debate over medical treatments and doping in endurance sports.

The link between lipedema, hormonal fluctuations, and gut health demands close attention from female endurance athletes. On Stemma Performance, sports nutritionist Marília Marques details estrogen metabolism and the risks of relying on medical shortcuts.

Lipedema is a chronic condition with a strong genetic and hormonal component that predominantly affects women. On Stemma Performance, sports nutritionist Marília Marques spoke with Bruna Mahn about how hormonal fluctuations and digestive balance drive inflammation, while also debating the implications of using medications and hormone therapies in endurance sports.

Estradiol fluctuations across life stages

According to Marília Marques, lipedema typically manifests or worsens during periods of intense hormonal shifts, especially those involving estradiol. These milestones coincide with puberty, pregnancy, and, most intensely, perimenopause. It is common for women who have always had fuller legs to experience increased pain, nodules, and inflammation specifically during the menopausal transition, when hormone levels fluctuate sharply.

Often, an athlete maintains a consistent training routine and disciplined diet, yet notices that lipedema-related adipose tissue does not respond like standard body fat. This frustration stems from the inflammatory nature of the condition, which is directly modulated by the hormonal environment.

The direct connection to gut health

A crucial factor in managing lipedema is gastrointestinal function. Estradiol produced by the body performs its physiological role, is metabolized and deactivated in the liver, and travels to the gut to be excreted in the stool. However, when dysbiosis—an imbalance in the gut microbiota—is present, this inactive estrogen can be enzymatically reactivated.

Once reabsorbed into the bloodstream in this reactivated state, the hormone displays high affinity for receptors in inflamed adipose tissue, fueling the lipedema cycle. For this reason, supporting the intestinal barrier and gut microbiome is essential for any meaningful clinical progress in treating the condition.

The endurance nutrition paradox

Maintaining a balanced microbiome presents an added challenge for female triathletes and long-distance runners. Endurance training requires high caloric intake and frequent replenishment with fast-absorbing, high-glycemic carbohydrates. When busy work and training schedules collide, many athletes turn to ultra-processed foods or quick fixes just to hit daily caloric targets, which can disrupt the gut ecosystem.

As Marques emphasizes, managing lipedema in athletic training is like solving a puzzle: athletes must meet the energy demands required to sustain heavy training without disrupting gut flora or exacerbating tissue inflammation.

Hormone therapies and the doping barrier

The debate surrounding pharmacological interventions is another sensitive topic. Options such as gestrinone (frequently associated with cosmetic hormone implants) are sometimes prescribed for aesthetics or symptom relief, but they are prohibited by anti-doping agencies. Bruna Mahn and Marques warn that athletes registered in official events—even in amateur divisions—are subject to anti-doping regulations and drug testing.

Choosing to use anabolic hormones or hormone blockers to manage lipedema is an individual health decision, but it is incompatible with competing in sanctioned sports.

Tirzepatide and the risks of training shortcuts

Another medication making headlines for weight management and inflammation is tirzepatide. While gaining clinical interest, its use in endurance sports demands extreme medical caution. Excessive appetite suppression can force an athlete to train in a severe caloric deficit, drastically increasing the risk of muscle and joint injuries alongside severe performance drops.

For Bruna Mahn, endurance sports prove there are no magic shortcuts. The safest path relies on an accurate diagnosis, multidisciplinary professional support, and the patience to honor the body's natural adaptation timeline.
