Can You Run and Do Triathlon with Lipedema? Here's How

Marília Marques and Bruna Mahn discuss pain management, inflammation control, and training adjustments for running and endurance sports.

A lipedema diagnosis does not necessarily mean giving up running or triathlon. Nutritionist Marília Marques explains how pain management, targeted nutrition, and multidisciplinary care make high-impact endurance sports possible.

A diagnosis of lipedema often comes with the fear of never being able to practice high-impact sports again. Initial medical recommendations to halt running are common during acute flare-ups, but the ability to train depends directly on the inflammatory stage, the patient's pain level, and how the condition is managed on a daily basis.

Pain scale and introductory sports

Lipedema is a chronic condition associated with tenderness to the touch, inflammation, and a persistent feeling of heaviness in the legs. As nutritionist and triathlete Marília Marques detailed, pain is typically measured clinically on intensity scales. When a patient experiences severe pain or mobility restrictions, running becomes immediately unfeasible, and water-based activities serve as excellent introductory sports to maintain conditioning without joint stress.

The role of nutrition in pain thresholds

For an athlete to sustain running or triathlon training—including Half Ironman distances—the initial step is reducing the pain threshold. According to Marília, nutritional interventions work directly on inflammation control. Without a targeted diet, proper supplementation, and, when necessary, medical treatment, training in higher-intensity zones (zones four and five) becomes unsustainable due to heaviness and burning sensations in the legs.

Workout muscle soreness versus lipedema discomfort

One of the main challenges in endurance sports is differentiating typical delayed onset muscle soreness (DOMS) from the inflammatory discomfort inherent to lipedema. Because athletic training produces transient physiological inflammation to stimulate adaptation, training with an already inflamed body amplifies the discomfort. As pointed out by Bruna Mahn, this overlap demands extra care with recovery and patience to avoid ramping up volume prematurely.

The multidisciplinary support triad

Living with lipedema and progressing in sports requires close coordination among a physician, nutritionist, and coach. The physician oversees diagnosis and medical therapies; the nutritionist fine-tunes anti-inflammatory strategies and body composition; and the coach periodizes training loads with the diagnosis in mind. Because lipedema affects both normal-weight and overweight women, aligning this triad is essential to enable safe and sustainable athletic performance.
